Notification of Readers Form (obtained from the Physics Graduate Office) 1 unbound copy (acid-neutral paper, 20 lb minimum weight required) 6 softbound copies (for 5 committee members and 1 outside reader)It is also requested that students submit 2 hardbound copies of the dissertation to the Physics Graduate Office - one for the Department and one for the Kline Science Library. This does not need to be done at the time of submission to the Graduate School.
The ``Notification of Readers Form'' that is prepared by the Physics Graduate Office lists the names and addresses of each member of the Committee and the outside reader. Therefore, the student must provide the Physics Graduate Office with the following information on the outside reader: name, title, address, telephone number, e-mail address.
The Graduate School then redistributes the final soft-bound version of the dissertation to the committee members with a Reader's Report form. This will be the Committee member's evaluation of the dissertation; therefore, it is imperative that what is submitted to the Graduate School has all the changes requested by the Committee. These completed forms are returned to the Graduate School within 30 days and are reviewed by the Graduate School Degree Committee before voting on the award of the degrees.
The Graduate School has very specific rules about the preparation of the dissertation, so students should obtain a Dissertation Packet from the Information Office, 140 HGS. This includes a copy of their booklet ``Preparation and Submission of the Doctoral Dissertation''. There are some forms to be completed and returned at the time of submission and fees to be paid. Current fees will be listed in the Dissertation Packet.
Note: Students must be registered through the term of dissertation submission (unless they have already completed their sixth year).
